**ALL PAPERWORK AND PAYMENTS MUST BE INCLUDED WITH YOUR APPLICATION for your application to be complete. Please do not send original veterinary vaccine records! Make a copy for us because we will not return what you submit.
REMINDER: NOCI does not accept electronic applications or attached paperwork. Application packets must be submitted via US mail or dropped off at the dog club.
HOW TO REGISTER A NEW DOG TO NOCI:
- You will need to complete the 3-page NOCI class application which can be downloaded here: New Dog Application. PLEASE PRINT WHEN YOU FILL OUT YOUR APPLICATION! We have received several illegible applications, and if we cannot read them, we cannot process them. Be sure to use an email address that you check regularly so you don’t miss important communications from us. Make sure you add to your contacts so it doesn’t get lost in your spam folder.
- Attach a copy of your most recent vaccination record from your veterinarian. We need a copy that is issued to you by your vet; writing in the dates of your vaccinations is not adequate for verification. Puppies must have had a minimum of their first two DHPP boosters prior to attending puppy class. Veterinarians are able to print a copy of your vaccine record upon request.
- Submit your payment. Our classes are seven weeks long, and the fee is $140 for the class. If your dog is from a rescue group, we offer a one-time discount of $70 for your first class when you provide a copy of your rescue paperwork. (Checks will not be deposited until your dog is enrolled in a class.)
- When you have completed all of your paperwork, including your payment, please submit it via US mail, or you can drop it off at the dog club. We accept checks or cash for applications for new dogs.
Please direct your application packet to:

5. When there is an opening in an upcoming session, the Director of Training, Ray Czubek, will contact you to arrange for an evaluation of your dog to determine if he/she is a candidate for a group class and to determine the most appropriate obedience class level. Evaluations take place the week before classes in a new session begin. THIS DOES NOT APPLY TO DOGS ENROLLING IN PUPPY CLASS.
